What Is Lipedema?

Lipedema is a chronic, progressive condition involving an abnormal and disproportionate buildup of fat tissue, most commonly in the legs, hips, thighs, and buttocks. It affects predominantly women, and in many cases has a genetic component.

Unlike regular body fat, lipedema fat typically does not respond to diet or exercise the way most patients expect. The fat accumulation is driven by a physiological process connected to hormonal changes, including those at puberty, pregnancy, and menopause.

Why Isn’t Lipedema Talked About More?

• Limited Research: detailed research has only recently been published.
• Frequent Misdiagnosis: lipedema is often mistaken for obesity or lymphedema.
• Stigma and Shame: many women feel embarrassed to speak openly about it.
• Lack of Medical Awareness: many physicians receive little to no training on how to spot the condition.

Recognizing the Symptoms

• Disproportionate enlargement of the legs, hips, or buttocks compared to the upper body
• Pain, tenderness, or aching in the affected areas, even without visible swelling
• A tendency to bruise easily in the legs or thighs
• A heavy or swollen sensation in the legs, especially after prolonged standing or sitting
• A nodular texture beneath the skin
• Skin that does not pit when pressed, unlike typical fluid-related swelling

Lipedema vs. Obesity vs. Lymphedema

Obesity involves excess fat throughout the body that typically responds to dietary changes. Lymphedema is swelling caused by a damaged or blocked lymphatic system and typically causes pitting. Lipedema affects both sides of the body equally, is associated with pain and tenderness, and does not respond to caloric restriction.

What Treatment Options Are Available?

There is no cure for lipedema, but Complete Decongestive Therapy (CDT) — combining manual lymphatic drainage, compression therapy, skin care, and therapeutic exercise — is the gold standard for non-surgical management. Lifestyle support such as anti-inflammatory or keto diets can also help. In later stages, water-assisted or tumescent liposuction may be an option for some patients.
