What Are Varicose Veins?

Varicose veins are swollen, twisted veins that form when blood pools in the legs rather than flowing efficiently back to the heart. Under normal circumstances, the veins in your legs use one-way valves to push blood upward against gravity. When those valves weaken or fail over time, blood flows backward and collects in the vein, a process known as venous reflux. The accumulated blood causes the vein to enlarge and bulge outward, eventually becoming visible beneath the skin.

While varicose veins are often dismissed as a cosmetic issue, they are a medical condition associated with a wide range of symptoms that can meaningfully affect quality of life. Left untreated, the underlying venous reflux driving them tends to worsen over time, and what begins as a cosmetic nuisance can progress to more serious complications, including skin changes, chronic wounds, and significant pain.

Symptoms of Varicose Veins

Not everyone with varicose veins experiences the same symptoms, and severity can range from mild to significantly disruptive. Common symptoms associated with varicose veins and the underlying venous reflux driving them include:

What to Expect During Your Varicose Vein Procedure

One of the most common concerns patients have before varicose vein treatment is not knowing what to expect on the day of their procedure. Let’s take a look at what a typical visit at Miller Vein looks like when you’re struggling with varicose veins.

Your procedure begins with a review of your treatment plan and any final questions with your provider. The treatment area is prepared, and local anesthesia is administered to ensure your comfort throughout. Most of our minimally invasive procedures are performed in well under an hour, and many are completed in under 15 minutes.

After the procedure, you are encouraged to walk immediately. There is no required bed rest and no need for general anesthesia or hospitalization. Most patients return to their normal daily activities the same day, with only minor activity restrictions such as avoiding strenuous exercise or heavy lifting for a short period following treatment.

Some bruising, mild swelling, or soreness around the treatment area is normal in the days following a procedure. These symptoms typically resolve within a few weeks. Your provider will walk you through exactly what to expect and what to watch for at your post-procedure follow-up.

Treatment Options

At Miller Vein, we do not take a one-size-fits-all approach. Every patient receives a personalized treatment plan developed by a board-certified vein specialist based on a thorough evaluation of their venous anatomy, symptoms, and goals. We use advanced diagnostic tools, including duplex ultrasound, to assess the severity and source of your vein disease before recommending any procedure.

In most cases, varicose veins do not need to be surgically removed. Instead, we use minimally invasive techniques to close the affected veins from within, naturally rerouting blood flow through healthy surrounding vessels. Here is an overview of the treatment options we offer.

Thermal Ablation

Thermal ablation is the most widely used and highly effective treatment for the venous reflux that drives varicose veins. It uses controlled heat energy, delivered through a thin catheter inserted into the affected vein under ultrasound guidance, to cause the vein wall to collapse and seal shut. Once closed, the body gradually reabsorbs the vein, and blood reroutes naturally through healthier veins nearby.

Miller Vein offers two types of thermal ablation:

Laser Ablation uses laser energy to heat and close the vein. A thin laser fiber is inserted into the problem vein, and precise pulses of laser energy are delivered along the length of the vessel, causing it to contract and seal.

Radiofrequency Ablation (RFA) uses radiofrequency energy delivered through the ClosureFast catheter to heat and close the vein. RFA is known for its consistent, controlled energy delivery and is one of the most studied and proven techniques in vein care.

Both procedures are performed in the office under local anesthesia, take under an hour for most patients, and allow immediate return to light activity.

VenaSeal

VenaSeal is a modern, non-thermal treatment option that closes varicose veins using a specially formulated medical adhesive rather than heat. During the procedure, a board-certified vein specialist inserts a small catheter into the problem vein and delivers a precise amount of the adhesive to seal it shut, rerouting blood flow to a nearby healthy vein.

Because VenaSeal does not use heat or tumescent anesthesia, the procedure requires fewer injections and is often more comfortable than thermal options. It is an excellent choice for patients who prefer to avoid heat-based treatment or who have anatomical considerations that make VenaSeal the preferred approach.

Sclerotherapy

Sclerotherapy, also known as endovenous chemical ablation (EVCA), treats varicose and associated veins by injecting an FDA-approved medication directly into the affected vessel. The medication causes the vein walls to close and the vein to fade from view. When a stronger effect is needed, the medication can be prepared as a foam, increasing its surface contact with the vein wall and improving its effectiveness on larger vessels.

Sclerotherapy is particularly well-suited for small to medium varicose veins, reticular veins, and residual veins following thermal ablation. Some patients require only a single treatment session, while others with more extensive vein involvement may benefit from additional sessions.

Microphlebectomy

For larger surface varicose veins that are visible and symptomatic, microphlebectomy offers direct, same-session removal through a series of tiny puncture openings in the skin. The incisions are so small that no sutures or stitches are required, and scarring is minimal. The procedure is performed in the office under local anesthesia and is often combined with thermal ablation or sclerotherapy as part of a comprehensive treatment plan.

Microphlebectomy is particularly effective for large, bulging varicose veins close to the skin’s surface that are causing significant discomfort or that are unlikely to respond fully to injection or ablation alone.
