But with the right program and the right team, both conditions are genuinely manageable, and meaningful improvement in your comfort, mobility, and quality of life is achievable.
Complete Decongestive Therapy (CDT) is the gold standard treatment for lymphedema and lipedema worldwide. It is the most evidence-supported, most widely recommended approach available, and it is the foundation of everything Miller Vein’s Certified Lymphedema Therapy program does for its patients.

Complete Decongestive Therapy is a comprehensive, multi-component treatment program designed to reduce swelling, improve lymphatic drainage, restore function, and equip patients to manage their condition effectively over the long term. It is the internationally recognized gold standard for the treatment of lymphedema and is also the primary evidence-based approach for managing lipedema and lipolymphedema.
CDT is not a single technique or a single appointment. It is a structured, ongoing program that combines several therapeutic components in a coordinated sequence, delivered by a Certified Lymphedema Therapist who assesses each patient individually and tailors the program to their specific presentation, goals, and stage of disease.
CDT addresses both the active reduction of existing swelling and the long-term maintenance of those results through patient education and self-management skills. This two-phase structure is one of the defining features of the program and one of the reasons it is more effective than any single intervention alone.
CDT is structured around two sequential phases that together address the full arc of lymphedema and lipedema management, from active decongestion to long-term stability.
The decongestive phase is the intensive, active treatment phase of CDT. Its primary goals are to reduce existing swelling as much as possible, soften areas of fibrotic tissue change, improve skin condition, and establish the foundation for long-term management.
During the decongestive phase, patients typically attend therapy sessions multiple times per week. Each session is intensive and purposeful, incorporating the core CDT components in a sequence and combination tailored to that patient’s specific presentation on that day. Your Certified Lymphedema Therapist assesses your limb measurements, skin condition, and tissue characteristics at every visit, adjusting the approach as the condition responds to treatment.
The decongestive phase continues until swelling has been reduced to a stable, plateau level. The duration of this phase varies considerably depending on the severity and stage of the condition, whether fibrotic tissue changes are present, how long the condition has been present and untreated, and how consistently the patient engages with the program between sessions.
Once swelling has been reduced to a stable level, treatment transitions into the maintenance phase. The focus shifts from active reduction to empowering the patient to sustain and build on the progress made during Phase One through a personalized, home-based self-management program.
The maintenance phase is not the end of the therapeutic relationship. Our Certified Lymphedema Therapists work closely with each patient throughout the transition, ensuring that the shift to home management is gradual, well-supported, and confident. We provide thorough training on every component of the home program and remain available for follow-up, reassessment, and garment adjustments as the patient’s needs evolve.
The maintenance program is built around five core elements: daily compression garment use, a home exercise routine, daily skin care, education on recognizing and responding to flare-ups, and guidance on activity modifications and lifestyle factors that affect the condition. Together, these form the framework that keeps swelling controlled and quality of life maintained after the intensive phase is complete.
CDT is effective because no single component does everything. Each element addresses a different aspect of lymphatic and vascular dysfunction, and together they produce results that none could achieve alone.
Manual lymphatic drainage (MLD) is the therapeutic cornerstone of CDT. It is a gentle, specialized technique that uses light, rhythmic strokes applied in a precise sequence to stimulate the lymphatic vessels just beneath the skin. The goal is to activate lymph flow, reroute fluid from congested or blocked pathways toward functioning lymph nodes, and reduce the accumulation of protein-rich fluid in the affected tissue.
MLD is profoundly different from conventional massage. The pressure is very light, the movements are deliberate and sequenced, and the technique is directed at the superficial lymphatic network rather than the deeper muscle tissue. It should only be performed by a trained and certified lymphedema therapist. In untrained hands, incorrect pressure or technique can worsen rather than improve lymphatic function.
Compression is applied to the affected limb throughout the CDT program to maintain the fluid reduction achieved during each MLD session and prevent fluid from re-accumulating between visits and after the intensive phase concludes.
During the decongestive phase, multi-layer short-stretch compression bandaging is typically applied after each MLD session. Short-stretch bandages work differently from standard elastic compression in that they create a firm, low-resting pressure that activates during movement and muscle contraction, providing the most effective support for the lymphatic and venous pumps in the limb.
As treatment progresses into the maintenance phase, custom-fitted compression garments replace bandaging for daily use. Selecting the right garment is an individualized process. Our CLTs evaluate each patient’s limb shape, swelling pattern, skin condition, and daily activities to identify the most appropriate garment in terms of compression class, fabric, and style.
Guided therapeutic exercise is an integral component of CDT at every stage of treatment. Specific movement exercises are prescribed to activate the muscle pump in the affected limb, stimulate lymphatic flow, improve joint range of motion, and support overall circulation. The exercises are designed to work with, not against, the compression that is being worn, maximizing the pumping effect on the lymphatic vessels.
Exercises are carefully matched to each patient’s current level of mobility and fitness and are designed to be safe, sustainable, and progressively built upon as the patient’s condition improves. Walking, cycling, and swimming are among the most beneficial general activities for lymphedema and lipedema patients and are often incorporated into home exercise recommendations.
The skin overlying lymphedematous tissue is at significantly elevated risk for infection because the protein-rich, stagnant fluid it contains creates an environment favorable to bacterial growth, and the compromised lymphatic system is less able to mount an effective immune defense in the area. Cellulitis is one of the most common and serious complications of both lymphedema and lipedema, and each episode can cause further lymphatic vessel damage that worsens the underlying condition.
Meticulous skin care is therefore not a cosmetic concern in CDT. It is a clinical priority. Our CLTs provide detailed, individualized guidance covering appropriate moisturizing products, inspection routines for catching early skin changes before they become infections, nail care practices, and general hygiene specific to the affected area.
Education is woven throughout every stage of CDT and is one of the most important investments made in each patient’s long-term success. Our Certified Lymphedema Therapists take the time to ensure every patient understands their condition, knows what triggers can worsen swelling, understands how to recognize the early signs of a flare-up or infection, and has the confidence to manage their condition effectively at home.
CDT is not something that can be approximated by general physical therapy, standard massage, or non-specialist swelling management. It requires specialized training in lymphatic anatomy and physiology, manual lymphatic drainage technique, compression selection and application, and the clinical assessment of lymphedema and lipedema at every stage of disease.
A Certified Lymphedema Therapist has completed post-graduate training specifically in lymphedema management that meets the standards of the Lymphology Association of North America or an equivalent credentialing body. This training includes both extensive theoretical education and supervised clinical hours specifically in CDT delivery.
The distinction matters because incorrectly performed MLD, inappropriate compression selection, or failure to recognize contraindications can worsen the condition rather than improve it. The quality of the CDT program depends entirely on the expertise of the therapist delivering it.

Starting CDT is a meaningful commitment, and we want every patient to understand what the process involves before they begin. Here is an honest picture of what to expect.
Your first appointment is a comprehensive evaluation with one of our Certified Lymphedema Therapists. We assess the affected limb or limbs in detail, take baseline measurements, review your medical history and the circumstances of your condition, evaluate your skin and tissue, screen for underlying vein disease, and discuss your goals and daily challenges. Based on this evaluation, your CLT develops a personalized CDT program tailored specifically to your presentation.
During the decongestive phase, you will typically attend sessions multiple times per week. Most patients begin noticing meaningful reductions in swelling within the first few weeks of consistent treatment, though the pace of progress varies with the stage and duration of the condition. Your CLT monitors your measurements and skin condition at every visit and adjusts the program as your response evolves.
As swelling stabilizes, the program transitions into the maintenance phase with a gradual reduction in session frequency and an increasing focus on home self-management. Our CLTs remain available for follow-up throughout and will reassess garment fit, home program effectiveness, and any changes in your condition as needed.
Commitment to the home program between clinic visits is one of the most important factors in CDT outcomes. Wearing compression consistently, performing your exercises, and maintaining your skin care routine every day are not optional add-ons. They are the difference between lasting results and gradual regression.
