Dr. Moorer welcomed Dr. Jeffrey H. Miller, MD, founder and CEO of Miller Vein, noting it was Cardiovascular Professionals Week. Dr. Miller was born and raised in Detroit, earned his medical degree with high distinction from Wayne State University School of Medicine, and a BS with high honor from Michigan State University. He is a board-certified interventional radiologist and phlebologist, a diplomat of the American Board of Radiology, the National Board of Medical Examiners, and the American Board of Venous and Lymphatic Medicine, and one of Michigan’s first physicians to perform minimally invasive venous treatment procedures.

He left a twelve-year hospital position to open Miller Vein, which now operates locations across Metro Detroit including Novi, Troy, Dearborn, St. Clair Shores, Macomb Township, and Auburn Hills. He has been named to Hour Detroit’s Top Doc multiple times, Corp! Magazine’s Entrepreneur of Distinction, Crain’s Detroit Best Places to Work, Detroit Free Press Top Workplaces, and several Detroit Free Press Best of Best Awards, including best vascular clinic for two consecutive years. He has cared for over 20,000 patients and partners with the Detroit Rescue Mission Ministries for free vein clinics.

Vein Disease as a Medical Condition

Dr. Miller explained that “leg pains, think veins” because vein disease is such a common cause of symptoms. He described the HASTY symptoms — Heaviness, Aching, Swelling, Throbbing, and Itching — as the most common signs of chronic vein disease, which is also a common, treatable cause of restless leg syndrome. Left untreated, vein disease can progress to skin changes and eventually leg ulcers.

Risk Factors

Dr. Miller identified four major risk factors for varicose veins: heredity (if both parents are affected, there is roughly a 90% chance their children will develop varicose veins), age, being female, and pregnancy, with each subsequent pregnancy increasing the risk further.

The Cardiovascular Connection

He described how arteries carry blood to the extremities and organs, veins carry it back to the heart, and the lymphatic system and skin are closely interrelated. When veins are compromised, the lymphatics and skin can be affected too, and fluid buildup under the skin can eventually lead to an ulcer.

Staying Active

For patients with limited mobility, Dr. Miller recommended movement whenever possible — even pumping the foot back and forth while seated — since the calf muscles push more blood out of the legs than any other muscle group. He also noted that obesity is a risk factor that makes vein disease worse, so better eating and more movement both help.

Restless Legs and Venous Reflux

Dr. Miller explained that a duplex venous ultrasound can identify leaky valves, and that 70–80% of restless leg syndrome patients with venous reflux feel better after treatment — sometimes allowing them to stop RLS medications altogether.

Building Miller Vein

Dr. Miller described how his interventional radiology group first got approval to use early radiofrequency ablation devices in 1999 to treat the underlying cause of varicose veins. As he became the go-to physician for vein procedures at his hospital, he eventually left his large radiology group — with his wife’s encouragement — to start Miller Vein on his own. Today’s minimally invasive procedures, including heat catheters and medical adhesives, typically take five to fifteen minutes, a dramatic change from the hospitalization and large incisions required decades ago.

Culture and Community

Dr. Miller emphasized putting employees first so they in turn take great care of patients, and described Miller Vein’s community partnerships, including free vein clinics with the Detroit Rescue Mission Ministries and “Operation School Bell” with the Southeast Michigan Assistance League, which provides clothing and essentials to local schoolchildren.

The interview closed with a discussion of Dr. Miller’s marathon training with his daughters and his philosophy of setting ambitious goals and working backward to achieve them.
